All North Shore Physicians Group primary care practices follow a Patient Centered Medical Home model. In a patient center medical home model, you, along with your primary care physician, lead a team of caregivers and other staff who work together to manage your care. This team approach enables you to have more time with your primary care physician and better coordination of care across several clinical specialties. In addition to your physician, your team many include:

You play an important part in this team-based model. Together with your physician you make the decisions that will be most beneficial to your health. NSPG is connected to the other hospitals within the Mass General Brigham network through a single electronic medical record. Care you receive at other hospitals or affiliated physicians will be reported back to your primary care physician and care team so that they can remain aware of your health.
To help us make the most out of every visit and to provide you with the most effective care, if you receive care outside of the Mass General Brigham network, please provide our office with timely information on:
- Your medications
- Visits to specialists
- Medical history
- Recent test results
- Self-care information
- Information from recent hospitalizations, specialty care, or ER visit
